This is the new CCGs Primary Care Response Model website.
Amongst other things it will contain up to date resources, useful for primary care on a range of subjects. These include, assessment algorithms, palliative care advice, predicative demand modelling, a summary of current evidence and much more. We are aware that there is a great deal of information circulating on social media and therefore the purpose of this site is to provide you with a clinically filtered version of the best information only.
The pages are designed to give GPs and PCNs as much steer and structure as possible. However, if you have a clinically sound local solution that meets the same requirements for your population it is perfectly acceptable. Clinical acumen and common sense prevail over guidelines.
Each of the pages is maintained by a group which, where appropriate, includes a range of system leaders and clinicians from the CCG, UHMN, UHDB, Local Authority, MPFT and NSCH, amongst others.
The primary care response model has also been signed off by our ICS footprint clinical leaders.

The website launched on the Monday 30 March 2020 and each of the groups is working hard to populate their pages. At launch you may find some pages are not fully ready, but the teams are working very hard to populate the pages with useful content. The website will continue to improve with your help and patience.
