One Health and Care Integrated Care Record

‘One Health and Care’ is a new integrated care record for individuals registered at Staffordshire and Stoke-on-Trent GP practices that brings data together from the different organisations involved in health and social care.

Completely confidential and secure, it’s designed to help doctors, nurses and other registered health and social care professionals directly involved in a patient or client’s care to make better, safer decisions.

Why you’ve received this information 

Following a fair processing campaign at the start of the year, we’re now looking to local stakeholders to help raise awareness of the launch of One Health and Care across Staffordshire and Stoke-on-Trent from this August.

You have received this toolkit because your staff, patients or clients are directly impacted by the launch. 

Why this information is important to you 

Essentially, the introduction of One Health and Care is a really positive change for everyone involved.  However, because it affects how someone’s personal data can be accessed, it is vital that it is communicated clearly and sensitively to your patients and clients.

This pack has been designed to equip you with all the tools to do this; from ready-made website and social posts through to an internal practice comms and FAQs for briefing staff.  There are also specific launch materials to support GP Practices.

Data being shared

 The One Health and Care launch will be phased, with data available from this August as follows:

Information Governance

You will be aware that a Fair Process campaign which included a GP Toolkit, Posters and leaflets ran for 12 weeks to make individuals aware that their data would be included in the One Health and Care Integrated Care Record. During this time if any individual wished to opt out they could do so by informing their GP Practice. The action for the practice is to update the clinical system with the appropriate opt out code.

Individuals can opt in and out of sharing data at any point, we ask that you explain the benefits of sharing appropriate data with those who are involved in their care and treatment to ensure the individual makes the right choice.

The project has a Data Protection Impact Assessment and an Information Sharing Agreement to ensure all IG and legal requirements are met.

All information viewed within One Health and Care by those accessing the system should be handled securely and confidentially in accordance with Data Protection legislation and your own practices local policies and procedures.

New Integrated Care Record (ICR) ‘One Health and Care’ – NOW LIVE 

There are over 120 Emis practices live with One Health and Care and the Microtest practice is also live.  GP practices using TPP as their clinical system will follow shortly.

With ICRs mandated by NHS England, it’s essential for your practice to understand what it is, what the benefits are and importantly, how to use it.

What is One Health and Care?

Completely confidential and secure, One Health and Care is an ICR for individuals registered at Staffordshire and Stoke-on-Trent GP practices that brings data together from the different health and social care organisations directly involved in their care.   

As a GP practice, you’ll now be able to access more information about your patients including details of any recent hospital visits and their latest social support summaries.​ Some electronic medical records that your practice holds on patients will now be visible to other health and social care professionals across the region too.

What are the benefits of One Health and Care?

FOR YOU 

  • You can provide better care by ensuring you have access to the most up-to-date information about your patients.​ As a result, this will help to improve your decision making.​
  • It reduces time spent obtaining or sharing information from other organisations involved in your patient’s care.

FOR PATIENTS 

  • They won’t have to keep repeating their health and social care history to different professionals.​
  • They’ll receive safer, more consistent care whether they are at your practice, hospital, or any other place where they receive care.

How do you access One Health and Care?

For EMIS and TPP practices, access is via single sign on (SSO). The One Health and Care Project Team will make contact with your practice to configure your clinical solution to access One Health and Care.
